Medicare Chronic Disease Management (CDM) Scheme
Enhanced Primary Care (EPC) items were removed from the Medicare Benefits Schedule in 2005. These were replaced by the Chronic Disease Management (CDM) items. The Chronic Disease Management GP services on the Medicare Benefits Schedule (MBS) enable GPs to plan and coordinate the health care of patients. Specifically patients with chronic or terminal medical conditions. A chronic medical condition is one that has been (or is likely to be) present for six months or longer. The definition for the CDM items includes musculoskeletal conditions.
Community-based patients may be eligible if they have a chronic (or terminal) medical condition. Noting that there is no list of eligible conditions. However, the CDM items are designed for patients who require a structured approach. Enabling GPs to plan and coordinate the care of patients with complex conditions who require ongoing care from a multidisciplinary team. A team in which physiotherapists play a key role in providing these CDM services. And a key role in the management of chronic musculoskeletal conditions.

Sydney Physio Clinic requires treatment fees to be paid in full. Following payment the receipt of payment can be taken to Medicare to claim your rebate. Sydney Physio Clinic does not invoice Medicare directly for Medicare physiotherapy services.
